Referring a friend or family member to our practice is the greatest compliment you could ever give to us.
We appreciate it so much that we’ll reward you both! Here’s how it works:
- Refer a friend for surgery and you will both receive $100 in rewards
- Refer a friend for Pure Skin procedures and you will both receive $50 in rewards
Referral rewards will be added to your patient account as a credit once your friend or family member has completed their treatment. There is no maximum - you may refer as many patients to our practice as you would like!

Please remember that we cannot share patient information due to HIPPA, so we will not be able to confirm if your friend or family member has visited the practice. Once you complete this form, we will automatically reach out to them with a brief welcome email with a quick explanation of our rewards.
