Helpful Information Before Your Appointment
Paperwork: Please complete your paperwork before your consultation. Your medical history helps us provide the best possible care.
Identification: We will scan your photo ID and insurance card at your appointment. Even if you are paying out of pocket, insurance billing may still be needed if a biopsy is required.
Questions: Your peace of mind matters to us. Please bring or write down any questions for your surgeon so we can address what’s important to you.
Planning: If you are considering multiple procedures, please let us know your top priority. If you have a special event coming up, tell us so we can help plan the best timing.
Cancellations: To cancel or reschedule, please give us at least 24 hours’ notice.
Payment: If a deposit is required, our front desk team will collect it. Please bring a form of payment.
